Hospital Bag Checklist for an Elderly Parent (Planned Admission)

A scheduled admission gives you something an ER never does: time to pack properly. This list is ordered by what the hospital asks for first, then the comfort items that quietly prevent the most misery.

Documents and copies — pack these first

ItemNote
1Photo ID copy + insurance cardsRegistration wants both; a clear copy is accepted at check-in.
2Current medication list (with doses)Admission medication reconciliation runs off this. The printed list beats a grocery bag of bottles — unless the hospital specifically asks you to bring specialty meds, which is worth one phone call before admission day.
3Allergy list, written downInclude the reaction, not just the name.
4Health care POA / health care proxy — clear copyHave it in the bag even if you expect to be alert and present. Not sure which documents count? See the POA & document locator checklist.
5DNR / POLST, if one existsBring the original signed form where you have it; a verbal description carries no weight.
6Insurance pre-authorization / referral paperworkWhatever the surgeon or scheduler told you to bring. Screenshot the email.
7Emergency contacts + the primary doctor's nameAdmission forms ask; hunting through your phone mid-lobby is avoidable.

Comfort and everyday items

ItemWhy it matters
Glasses + case, hearing aids + spare batteriesDays in bed without them read as confusion, not sensory loss.
Dentures + case, and the adhesiveEating depends on it; nurses rarely hunt for it.
Nonslip socks or comfortable slip-onsThey will be walked somewhere. Bare feet on a hospital floor is how falls happen.
Phone charger with a long cableOutlet placement is never next to the bed. Long cable, every time.
Robe or cardigan, lip balm, lotionHospitals are cold and dry; small comforts get used hourly.
A notebook or the daily logNames of the care team, questions, what each doctor said. See daily care log.

How many days of clothes

Plan for the scheduled length plus two days — discharges slip — but keep it to three or four outfits total. One going-home outfit stays folded in the bag; nobody wants to leave in a gown that ties at the back. Hospital laundry rules vary, so ask rather than assuming family can wash things.

Leave at home

The night before

Different problem? If the trip is unplanned, speed beats packing: what to bring to the ER — checklist covers the grab-and-go version of this list.
