Medication List Template: What It Must Include (and What Ruins One)

The medication list is the single most-read page in any caregiver binder. Here is what earns its keep.

The columns that matter

The four mistakes that make lists useless

  1. Skipping OTC and supplements. Ibuprofen with blood thinners, fish oil before procedures, St. John's Wort with antidepressants — the interactions live exactly in what people leave off.
  2. No dates. A list with no "updated" date is a rumor. Add the date; retire anything stopped over three months ago into a short "recently stopped" footnote — recent changes are the first thing discharge teams ask about.
  3. Transcribing from memory. Read the bottles. Photo of each label beats recollection, every time.
  4. Allergy listed without the reaction. "Penicillin — hives" and "penicillin — throat swells" are different risk levels and different paperwork.

Where the list lives

Three copies, one job each: in the caregiver binder (the master), one in the ER bag, one on the fridge for anyone who steps in mid-crisis. Update all three together or the wrong one wins. A daily log records what was actually given — the list records what is supposed to be given; doctors want both.

The CareBinder generator prints the medication table formatted like the one above — dose, schedule, purpose, prescriber — alongside allergies, insurance and everything else an ER wants. Free, in the browser, nothing uploaded.
